61 Y/O F during an evaluation for a kidney transplant a very high PTH was noted 1900 pg/ml N was noted. A parathyroid scan showed uptake in the chest by arrow. 99mTc sestamibi was injected and the hyperplastic parathyroid mass removed.

Ectopic parathyroid tissue and adenomas are rare. They can be located anywhere along the descent of the parathyroid glands from the high neck to the mediastinum. When in the chest they may be attached to the thymus. Some can be found as low as the dome of the diaphragm.

Image is sideways but the arrow is on the ectopic parathyroid to right. Uptake is seen in the noprmal thyroid gland and salivary glands to the left.
