Spot urine iodine is not a good test for iodine deficiency due to marked shifts during the day. It can only be used in population studies.Beware of physicians doing this spot iodine test on you. Severe iodine deficiency causes cretinism,hypothyroidism goiter deaf mutism, and loss of IQ. It is the leading cause of IQ loss in the world. Universal iodization of salt is the best way to deliver iodine to the pregnant. 50-150 mcg /day mild to moderate deficiency has been linked to ADHD, goiter, small head size and decreased placental weight. Breast feeding in the USA may have a group of women with moderate deficiency but no data available. Iodine supplements during pregnancy is common in the US but there is no hard data. 250 mcgs of iodine per day is recommended for pregnant women. Planning pregnancy should include 150 mcgs of KI for 3 months before.There is no need for extra iodine supplements if the patient is on thyroid hormone or treated for hyperthyroidism, but the evidence is weak. Excessive iodine during pregnancy above 500 mcgs is dangerous for the fetus.
