  1. Get a copy of the thyroid ultrasound from your primary.
  2. Read the report on your own. See if there was a big nodule with a cyst fluid component.
  3. If the biopsy of the nodule was benign you could be a candidate for PEI.
  4. If you have symptoms from the nodule or it is a cosmetically ugly neck mass you are a candidate for PEI.
  5. Your primary physician will usually recommend surgery.
  6. Confront them about your desire not to have surgery and you have heard about PEI as a first line alternative to surgery.
  7. PMD may tell you it is dangerous and painful. Do not believe it as surgery is much more likely to cause complications than PEI.

  12. The evaluation is to make the risk of cancer < 3% by a repeat second biopsy by me and a molecular marker and classifier, detailed endocrine neck ultrasound and repeat thyroid function tests including antibodies and growth marker thyroglobulin.

  16. The patient refused surgery for years with this parathyroid cyst. After years she had PEI in my center. 13 ml removed and 6 ml ethanol injected. The cyst was ablated as seen in the lower US images. PEI is good for thyroid or parathyroid cysts.

  18. One needle is inserted into the cyst under ultrasound guidance. The cyst fluid is removed and the amount s measured and the injection of 200 proof medical grade ethanol is determined as 50% of the fluid removed.
  19. The ethanol is left in for 5-10 minutes and removed.
  20. There is only slight pain with the removal of the ethanol along the needle tract.
  21. Complications of a serious nature is rare. Side effects are self limited and short term.
  22. You leave with your thyroid gland intact, and no need for thyroid hormone therapy after surgery.
  23. If this sounds too good to be true it is too good.
  24. No surgical complications of nerve damage, or parathyroid dysfunction, No scar, no surgery hangover, two week recovery, no pain medications.
  25. Costs are 1/10 of hospital surgery.
