Professor Jung Hwan Baek’s Textbook on Thyroid RFA for Nodules and Cancer: A Review

Dr.Guttler’s Review:

  1. Professor Baek’s 40 page test just released on the internet is a concise work covering the history, physics, indications, procedures, efficacy, safety, and Korean training programs.
  2. The history section is only about Korean presentation experience.
  3. A one page summary of the physics is supported by 2 drawings and is helpful.
  4. elec-effect
  5. gross
  6. Necrosis of the nodule after RFA.
  7. The moving shot technique is shown to be superior to fixed electrode method.

  10. Indications section includes consensus statements on thyroid nodules and cancer from the Korean Radiologists. This includes the correct devices to us for nodules and cancer. And the safe way to approach the nodule or cancer with a trans-isthmic insertion of the electrode. The size of the electrode “hot spot” is adjusted from # 10 mm to 5 mm for cancer lymph nodes and tight areas near the danger triangle.probes
  11. The electrode is either #18 or 19 gauge. He describes the use of a virtual needle tracking system to improve location finding within the nodule.
  12. tracker
  13. Another new technique is using a dedicated needle guide for moving shot technique.
  14. that is not fixed and moves to different areas of the nodule. This is a breakthrough and superior to free hand method.
  15. The largest chapter is on efficacy and safety for thyroid nodules, cysts, AFTN
  16. toxic

  18. big-goider
  19. recurrent papillary thyroid cancer in neck lymph nodes

  21. , and 4 year follow up of results. A comparison with laser showing RFA was superior.Innovative devices and technique is an excellent section. It includes ways to ablate vessels that leach heat from the electrode, called vascular ablation before the RFA

  23. . The local injection of the capsule with lidocaine has helped pain control and is all you need for RFA. The use of dextrose drip between the target and the danger structure is called Hydrodissection.

  25. Dr.Baek and his team have been in the forefront of finding new ways to make RFA safer for thyroid nodules and cancer.
  26. Complication section covers the main minor and major complications with thyroid RFA. 3.3% total complications and 1.4% majors with rare sequelae rate of 0.14%. Voice change, hematoma vomiting, skin burn are the most common. Rare nodule rupture abscess, and hypothyroidisn are noted. one case of transient brachial plexus injury was reported. All were the temporary except the abscess needed surgery and the hypothyroid needed thyroid hormone therapy.

  28. Personal plug for his training programs at his Asan Medical center.hospital. There are 3 and 7 day courses with daily protocol listed in the book.
  29. Chapter on references and on web based CAD system for thyroid ultrasound nodule diagnosis complete the textbook.
  30. Professor Baek includes homepage for his  thyroid lab (www.gap.kr). and his research Gate website www.researchgate.net/profile/Jung_Baek2.
  31. Conclusions:Anyone interested in doing Thyroid RFA or learning about it to refer to experts will find this small gem invaluable.
  32. 5 Star review.
