Since Dr.Jack Baskin pioneered the use of ultrasound and spearheaded the formation of the American College of Endocrinology’s endocrine certification in neck ultrasound called ECNU hundreds of endocrinologists have been certified. At his Thyroid center in Orlando Florida he personally trained endocrinologists in the use of US guidance to treated thyroid cysts and cancer neck lymph nodes not yet palpable. I spent a week with Jack and he taught me ultrasound and procedures using ethanol to ablate cysts and cancer lymph nodes. Since 1990 US has become an major part of thyroid disease evaluation.He pushed the radiologists to begin to have their sonographers look beyond the thyroid to other structures related to thyroid and parathyroid disease. It was common to see a large cancer lymph node un-reported by radiology, right next to a thyroid nodule. The nodule FNA was suspicious but the lymph node was positive for cancer by cytology and needle washout for thyroglobulin TG. He also taught me ethanol ablation of parathyroid and thyroid cysts. Recurrent lateral neck lymph nodes were seen on neck ultrasound and could be treated in his office with ethanol ablation instead of expensive repeat modified radical neck dissection.Parathyroid adenomas are now visualized by Neck ultrasound.Questionable ones near or in the thyroid or finding a thyroid nodule are indications for FNA. The addition of needle washout for TG and parathyroid hormone PTH to the FNA can settle on if it is thyroid or parathyroid. He was an early supportor of ablation techniques with laser and radiofrequency for solid benign nodules being done by AACE member Dr.Roberto Valcavi in Italy and others in Korea.
