  1. Bethesda Reporting system new update has three ways to handle indeterminate thyroid nodules.
  2. Careful evaluation of every biopsy to be sure the diagnosis is correct.
  3. Too many biopsies are done in an improper manner resulting in inadequate biopsies with blood induced artifacts to be called indeterminate.
  4. Repeat all biopsies with an indeterminate result to obtain molecular markers.
  5. Option three is to use observation on some ultrasound low low risk nodules instead of biopsy and surgery.
  6. BRAF driven Classic Papillary thyroid cancer is a direct to cancer lesion. No pre-cancer.
  7. RAS driven follicular lesions including Follicular variant of papillary cancer, follicular adenoma, follicular cancer and NIFT-P follow a pattern of adenoma to cancer like other cancers in the body.
  8. Non-invasive follicular tumor with papillary like cells NIFT-P has extremely low risk of conversion to cancer and can be treated with just a lobectomy and no cancer surveillance post op.
