  1. The key finding reported was the decline in surgery for primary therapy of papillary micro-cancers.
  2. “Less is more”  is also the theme for larger thyroid cancers.
  3. Follicular variant papillary thyroid cancer and NIFT-P are treated with lobectomy not totals.
  4. Classic papillary < 4 cm also may be treated less aggressively with lobectomy.
  5. Total thyroidectomy now is only for more aggressive cancers.
  6. Less central neck dissections are also recommended now.
  7. Pre-op ultrasound localization of cancer lymph nodes is key to deciding the extend of the first surgery.
  8. The key word is “de-escalation of the use of thyroid surgery in 2019.
  9. Second opinions are necessary to protect the patient form knife happy surgeons who do not want to lose a patient to a different opinion that may not recommend surgery or direct the patient to a high volume surgeon who may do a less aggressive surgery.
