After total thyroidectomy, right modified radical neck dissection, and 2 courses of radioiodine a single abnormal lymph node with abnormal blood flow was noted on follow up endocrine neck ultrasound shown below.
After FNA and needle washout for thyroglobulin confirmed the cancer, Ethanol was injected into the lymph node after confirming that the nodal capsule was not invaded by a small injection of 0.025 ml and asked if there was any pain. If not the node was ablated with 1.0 cc 200 proof medical grade ethanol.
