What to Expect When You Go for a Thyroid Radiofrequency Procedure?

  1. The RFA candidate arrives for the procedure without pre-medication.
  2. Removes all metal jewelry, watches, and belts etc.
  3. A paper gown will be used.
  4. The patient will be lead into the Ultrasound room which has been prepared for thyroid RFA.
  5. The patient will lay face up on the examination table.No IVs are needed.
  6. Vital sign monitoring is available only if needed.
  7. The back of the head will be placed on a donut and in a slightly elevated position.
  8. The size and location of the nodule will be marked under US guidance.
  9. Dr.Guttler will inject lidocaine in and around the spot marked for entry of the needle.
  10. After a few minutes he will inject lidocaine around the thyroid capsule. The capsule is the only place there are sensory nerve fibers causing pain. The inside of the thyroid and the nodule have no nerves causing pain.
  11. You will be awake and be able to let me if you have any symptoms.
  12. There is a mirror on the ceiling so you can follow the RFA procedure.
  13. Dr.Guttler will now introduce the needle with a small 10 mm tip that can be activated by foot pedal to produce heat to ablate sections of the nodule.
  14. Depending on the size it may take 10-30 minutes to ablate your nodule.
  15. Lymph nodes, parathyroid, and micro-papillary cancers are small and will take lesser time.
  16. If you have any pain, I will reduce the amount of heat being delivered.
  17. You will stand up and leave the ultrasound room and after a few minutes if you do not have any symptoms of bleeding you can leave and go home or back to work with only ice packs and Tylenol for post RFA treatment.
  18. Dr.Guttler will see you the next morning to review the ultrasound for ablation.
  19. You will be seen at 1,3,6, and 12 months to seen if you need another treatment session.
  20. As you can see this is a simple procedure and can replace the hospital stay and surgery with scar and the need for thyroid hormone therapy for life.
