Thyroid Nodule 101: Norville Case is a Wake up Call to Primary Physicians: Look at the neck when doing the physical or face the patient’s rath when the cancer was discovered by a fan instead of them.

  1. Deborah Norville’s primary missed the nodule that was picked off TV by a fan.
  2. If it was big enough to see off TV it was big enough to be picked up by her PMD earlier.
  3. Why did this happen?
  4. The primary did not follow the simple rules of thyroid examination.
  5. Rule one is to watch the neck while the patient lists the symptoms to see any abnormal movement.

  7. After observing any irregular neck structure movement feel the neck later during the physical.
  8. Most hurried PMDs examine the neck while the patient is sitting. This leads to missed diagnosis as the thyroid can drop into the upper chest. Some exam from the front and some from the back.

  10. The thyroid is best examined when they are lying down.
  11. Examine the neck from the head of the table. There are few images on google showing the proper way to examine a thyroid. This is the closest example of a supine exam of the leg. To do a thyroid exam supine put the pillow behind the neck and stretch the neck back over the pillow. Many nodules and enlarged goiters are missed by sitting method.

  13. Using this simple approach the primary physician will not face the sad fact that a fan watching TV found her nodule before her own physician.
