Thyroid Tumor Ablation Course in Korea in 2018
In May 2018 I have the honor of being invited to Seoul Korea to attend a special workshop with Professor JH Baek at Asan Medical Center. Dr.Baek is world expert in thyroid tumor ablation with ethanol PEI and radiofrequency ablation RFA methods. Soon RFA will be approved for treatment of thyroid tumors in the USA. I will be active in this introduction. First as a primary source of RFA training of endocrinologists needed to do the initial clinical trials, and with the task force on thyroid ablation of the American Association of Clinical Endocrinologists to begin the process of training and certification in thyroid tumor ablation with RFA. I am honored to be included in the task force. I have been working on RFA for 3 years and with two different systems. The present system has a thyroid friendly short narrow gauge electrode. This is specially design for the small space in the neck.
The task of teaching RFA before live patients is done by a series of models. The actual ablation procedure is done with either bovine liver or superconcentrated gelatin stuffed with beef “nodules”.

Results of simulated beef nodule ablation with gelatin model.
Similar results using bovine liver is used for teaching in Korea and Europe.
Electrode showing the 10 mm burn area on the surface of the bovine liver.
Various techniques also have models to simulate each procedure needed during actual RFA of thyroid nodules.
Thyroid capsule Lidocaine injection is simulated by

Actual thyroid capsule Lidociane injection.
Simulated Hydrodissection “Dextrose Avoidance” system using models.
Dextrose is infused between the target and the danger structure
Actual Hydrodissection Method to move dangerous
structures away from target nodules.
Teaching techniques include a needle tracker
Bipolar electrode for pregnant and pacemaker patients.
Half electrode with heat for close areas near danger structures.

Thyroid tumor ablation Master’s Class is open for endocrinologists with extensive experience in doing their own USGFNA. We need a few good people like Dr. Brittany Henderson to learn RFA ahead of the FDA approval.
The fee for expert FNA endocrinologists will be reduced based on skills and the desire to be part of the clinical trials on thyroid nodules in the near future.
