Dr.Guttler’s comments:

  1. Usually these are handled by injecting a small amount of ethanol and repeat the ablation in a few weeks. This is called a 2 step PEI.
  2. This article on a one step for those nodules.
  3. A 16-gauge needle (n = 8) or an 8.5-French pigtail catheter (n = 1) connected to a suction pump was used.
  4. 6 month nodule size was reduced 96%. The mean symptom score decreased from 3.2 ± 1.5 to 0.4 ± 1.0 (p < 0.05) and the mean cosmetic grade from 3.9 ± 0.3 to 1.3 ± 0.7 (p < 0.05).
In a small sample size (10 patients ) this one step method has promise. However the needle size is a problem. However they report no complications.
  5. If you have a thyroid cystic nodule call me for an evaluation for ethanol ablation instead of surgery.
