  1. When your primary tells you to skip seeing an endocrinologist or thyroidologist and go direct to have a biopsy and then to the surgeon say no and seek a endocrine consultation.
  2. The quality of the radiologist biopsy and the local pathologist diagnosis varies from poor to adequate.
  3. Case in point. Patient had a very small nodule (10 mm ) biopsied and told she was high risk for thyroid cancer and needed surgery.

  6. Review of the biopsy found severe excess blood due to bad technique of the radiologist doing the biopsy, and worse the pathologist. The blood caused distortion of the cells stuck in blood and fibrin causing over-diagnosis of  follicular neoplasm.

  8. I told her she did not need surgery but a “good” quality  repeat biopsy and molecular markers. Repeat was called benign and the markers were benign.

  10. The markers confirmed that the nodule was benign, with benign classifier and no DNA cancer markers.
  11. Before allowing a biopsy confirm it is necessary by visiting an endocrinologist first.
  12. Thyroid surgery is a major operation with many unwanted side effects and bad complications.
  13. If it is really needed it is worth the risk however,if it was done for the wrong reason it was not worth the risk.
