Surgery for Thyroid cysts 101: 10 year post PEI No Recurrence.
10 years ago I saw this 31 Y/O male patient from China.He was unable to get alternative therapy for his nodule in China. Traveled to my center. He had a 7 mm nodule for 5 years before he saw me. He had no local symptoms but the location and the visible aspect made him seek my care. He refused surgery in China. Thyroid biopsy was benign. Thyroid imaging at that time showed a cold nodule in the left lobe.
My ultrasound : 2 cm complex cystic nodule.
The first PEI failed to remove the cyst fluid. A two stage PEI was then the next step.
A < 0.5 ml ethanol was injected in the center of the cyst. This was to loosen the thick fluid. He returned in a month and then the second PEI was successful. 2 ml brown cyst fluid removed and 1 ml 200 proof ethanol injected and removed. The patient slight neck pain on needle withdrawal only.The procedure took 10 minutes.
5 years the cyst was < 2 mm
10 years the cyst was completely ablated.
