Thyroid No Surgery:Thyroid Ethanol Ablation 101: 10 Year Follow up of Thyroid Cyst Treatment.
37 Y/O Male RZ comes to see me when he could not get alternative therapy for his thyroid nodule in Singapore China which was first noted 6 years before he visited the US to see me.. He found me on my thyroid.com website. Except for the nodule he had normal thyroid blood tests including thyroid antibodies. He had a negative family history for thyroid disease and cancer and no radiation exposure. The thyroid was visible on neck inspection on the left. Palpable firm nodule was felt on the left. Endocrine neck Ultrasound was negative except for the absence of the congenital absence of the isthmus and the complex cystic nodule. Am insignificant 2 mm nodule was seen in the right lobe. The left nodule was 1.5 X 1.4 x 2.5 cm ( 3 .1 ml volume).
USGFNA of the nodule was negative. colloid nodule.
The patient had ethanol ablation after removing the cyst fluid. Except slight pain on withdrawal of the needle the procedure was uneventful. There was no residual nodule seen by 18 months post PEI treatment on ultrasound. 10 year follow up ultrasound still showed no recurrence.
I have been doing ethanol ablation since early 2000 and this is an example of how I use alternative ablation procedures such as PEI and radiofrequency ablation RFA to replace costly surgery in benign conditions. RFA is still not available in the US but I send my patients to Italy or Korea for RFA if they are willing to fly. Long term follow up is needed to see the success for PEI therapy as noted in this 10 year follow up with no recurrence in patient RZ.
