Thyroid RFA 101: Cosmetic Issues Related to a Big Benign Goiter Nodule Treated Without Surgery
57 Y/O F hx of a goiter since 2010. She does not want surgery.She has cosmetic issues with the goiter and does not like the way it is pushing her air pipe over to the right. She has no significant local neck symptoms and has normal thyroid tests including TSH 3.7. PE visible goiter from across the room ( first picture). Tracheal deviation to right away from the larger mass on the left.
Prior thyroglobulin was 300 in 2010 and now 444 consistent with growth.
Pre-Thyroid RFA The visible mass on the left can be seen pushing the trachea to the right.
After evaluation at my center including a negative second biopsy and molecular markers she was sent to Italy to Dr.Roberto Valcavi for a short 2 day visit just to treat the goiter.
Centro-Palmer Institute includes hotel and transfers to and from the center for the treatment and one day follow up.

A 50 ml nodular mass in the left lobe and a smaller nodule on the right was ablated. She had no symptoms or complications from the RFA treatment. Post thyroid RFA she took tapering dose of steroids to prevent edema or inflammation.
One year post RFA She returns very happy with the response.The cosmetic issue of hers was treated.
She still has a goiter and needs yearly follow up. The nodules were decreased by 50%.The trachea was returned to midline. She is not interested in complete removal, but knows she needs follow up in the future.She was informed the nodule may grow and she would need another thyroid RFA treatment.
