Need Thyroid Surgery? What are the risk factors for a Bleed post op?
Lui J. et al European J.Endocrinology
25 studies 46,000 patients with 6600 with post op hemorrhage 1.4%. Increased risk factors were older age, male sex, Graves’ disease, anticoagulant or antiplatelet therapy, total thyroidectomy, neck dissection, and previous thyroid surgery. Malignant tumor, and post op drainage device.were not associated .
Comment: If you need thyroid surgery be sure to have a complete extensive pre-op evaluation for these risk factors before the surgery. Even a 1% risk of a possible fatal bleed is worth your time to make sure you are at the best hospital with a dedicated area for neck surgery with nurses who know when an emergency bleed is happening. An emergency trach can save your life. I had one male singer who had this happen at Cedars /Sinai in LA, and had a rapid trach and when he saw me 6 weeks after the trach was out and he was doing well.
