Private course for only one endocrinologist or ENT ( 2 by special arrangement). Thyroid RFA for thyroid nodules.

This is a hands on course with Dr.Guttler at your side for 2 days. This is intensive personal training with 4 hours each day using a RF Medical RF system to ablate “beef nodules” in a gelatin or bovine liver models. The usual courses have too many students and have less time on the generators.

Day 1

1. Ultrasound Anatomy of the Thyroid, neck, vagus, recurrent nerves, esophagus and trachea.

2. RFA Guidelines Present Korean Position papers on use of Thyroid RFA.

3. Pre-procedure: equipment,  knobology of generator, types of electrodes, consent,

questions on symptoms and cosmetic issues, Cooling system, Dextrose IV drip.

Evaluation: H&P, Neck Examination, US nodule volume, arteriole location near the nodule, looking for the vegas, and blood vessels in the isthmus

4. Case Observation and RFA hands  on with supersaturated gelatin and Bovine liver phantom.

Safety of radiofrequency ablation of benign thyroid nodules and recurrent thyroid cancers: a systematic review and meta-analysis.

Operator experience, not all used trans- isthmic approach, some used old type hook electrodes



1. Physics of RF ablation Thermal heat production, friction heat and conduction heat.

2. Postprocedural Care & Management of the patients; Observation for 30 minutes, 1,3,6,12,24 month Follow up with neck ultrasound.

3. Key Techniques: capsule Lidocaine injection, Dextrose infusion Microdissection technique, Arteriole ablation near target nodule. Bipolar electrodes, 2-10 mm hot tips for various targets, #18 and 19 electrode needles, and needle tracking systems for location of the hot tip at all times.

4. Post-procedural Follow up.

*** The number of case observation can be varied by the volume of cases each week, but at least a single case is guaranteed including all processes of a preprocedural interview, US scan & planning, patient preparation, manipulation of RFA equipment, pain control, ablation, and postprocedural care.

2.4 hours of hands on Thyroid RFA practice with phantoms.

The course can be expanded to 3 days to include Ethanol ablation of nodules cysts and cancer lymph nodes.

For endocrinologists who don’t do biopsies an additional 1-2 days would cover everything up to the ablations. Neck ultrasound, USGFNA, needle placement, sampling techniques, slide making, on-site adequacy, needle washout methods for Cancer markers.

Five days would get you up to speed doing clinical thyroidology.
