Thyroid Cancer No Longer: What is a NIFTP, non-invasive follicular tumor with look alike papillary thyroid cells?

 

  1. It replaces a cancer called non invasive encapsulated follicular variant of papillary thyroid cancer, EFVPTC.
  2. 10-25 % of thyroid cancer patients have this NON-CANCER.
  3. Patients with NIFTP do not have cancer.They avoid completion surgery and radioiodine.
  4. Surgery is still needed to be sure the benign tumor has no capsule invasion.The entire capsule is studied after the surgery.
  5. Present FNA and mutation studies can not replace surgery.
  6. Lobectomy is the surgery of choice.
  7. Long term “cancer” followup is not needed.
  8. They do not carry a diagnosis of thyroid cancer.
  9. Size does not matter in NIFTP. Even large >4 cm lesions show no signs of lymph node spread. It is just a large benign tumor period.
  10. NIFTP lacks papillae psammoma bodies,infiltration of capsule, and no necrosis.
  11. NIFTP have 0% recurrence rate.
  12. 40,000 + vases a year will avoid a cancer diagnosis, completion thyroidectomy and radioiodine therapy.
  13. Impact will be enormous for those patients as they will not carry a cancer diagnosis.
  14. The surgery is needed but there is hope for mutation studies to be able to pre-op diagnose NIFTP.
  15. If you have a follicular varient of papillary thyroid cancer before you decide on the surgery make sure it is not a NIFTP. The surgery should be limited to a lobectomy only not a total. A benign tumor should leave you with a functioning lobe and may not need thyroid hormone, completion thyroidectomy or radioiodine therapy. You are cured and do not have cancer.
