Wake Up Call to Physicians Sending Benign Thyroid and Parathyroid Cysts to Their Local Surgeons:
American Association of Clinical Endocrinologists AACE, and the American College of Endocrinology in association with a European endocrine society in their task force on thyroid nodules headed by Dr.Gharib from Mayo clinic stated:
“Percutaneous Ethanol Ablation PEI should be the first-line treatment option for relapsing,benign cystic lesions.”
Why risk morbidity,complications and expense of thyroid surgery in the hands of the majority of low volume surgeons <25 total thyroidectomies a year when there is an approved non-surgical alternative available. Why has it taken so long to get recognition for the value of a simple outpatient procedure to treat thyroid and parathyroid cysts? In my opinion, when all studies including careful ultrasound guided biopsy of all solid areas on the wall of the cysts and molecular marker and classifiers are negative, PEI is the treatment of choice for relapsing benign cystic thyroid nodules with >50% cyst fluid.It is the physician’s duty to look for an interventional thyroidologist/radiologist near them to refer their patient instead of to the surgeon.
