What to do with <1 cm nodules PMT papillary microcarcinomas that are candidates for Active Surveillence? To biopsy or not that is the question.

Rosario et al Diag.Cytopathology Feb 20 2017

FNA was done on <1 cm nodules that had suspicious features on ultrasound and possible candidates for active surveillance. 198 nodules. 40% were benign on FNA. All 90 suspicious nodules or malignant on FNA were cancer at surgery. 22/27 indeterminate were malignant or 82%.

Conclusion: FNA changed the presumptive diagnosis of PMT in 30% of the nodules. FNA may be needed to down grade a presumptive diagnosis of PMT  to benign.
